MEDICAL RELEASE OF LIABILITY

I understand that persons engaged in dance related activities may be at risk for injuries related, but not limited to death, serious neck and spinal injuries, brain damage, internal organ,bone, ligaments, muscles, Sprains of joints and tendon, damage. I hereby assume all risks associated with my child/myself participating in dance class activities or affiliated outside activities and performances, and agree to hold harmless J. Rene Academy of Dance/ Performing Arts Center, LLC, and any of it's agents or representatives from any liability, actions, causes of actions, debts, or claims of any nature whatsoever which may arise by or in connection with our participation in dance class or outside related performance activities with J. Rene Academy of Dance. The terms hereof shall serve as a release and assumption of risk for my heirs, estate, executor, assignees, and all members of my family. I will assume the cost of ambulance services in case of emergency. I grant authorization and permission to the supervising Teacher/Director/Instructor in order that my child/children may receive the necessary medical treatment in the event that she/he may sustain injury or illness during participation. I understand that physical contact and corrections are needed in artistic dance technique and instruction and give my permission for this element of the class. I understand that J. Rene Academy of Dance has behavior and safety rules and that my child will not be allowed to participate if said rules are broken or if they put another person(s) in harms way. I will be responsible for her/his failure to abide by the rules and or upon injury to another student. J. Rene, LLC has the right to terminate student instruction and refuse service at any time for any reason.
  Payment Agreement:

I AGREE TO PAY FOR SERVICES ON A 30 DAY CONTRACT BASIS AND UNDERSTAND I WILL BE CHARGED FOR THE LAST MONTH UPON MY WRITTEN NOTIFICATION BY THE 5th OF PRIOR MONTH ALONG WITH ANY OUTSTANDING LATE FEES OR OTHER COSTS ACCULUMATED AT DROPPING AND THAT NO FEES ARE REFUNDABLE. I AGREE TO ALLOW J. RENE ACADEMY OF DANCE TO AUTO-BILL MY CREDIT CARD THROUGH THE PAYSIMPLE RE-OCCURING PAYMENT PLAN THE 1ST to 5TH OF EACH MONTH AS REQUESTED ON MY AUTHORIZATION FORM. I AGREE TO PAY ANY FEES ASSOCIATED WITH DECLINED PAYMENTS $20 FOR FAILED PAYMENTS WILL BE ADDED TO FOLLOWING MONTH CHARGE. ANY OUTSTANDING FEE'S ASSOCIATED WITH DANCE WILL BE CHARGED TO ACCOUNT AFTER 7 DAYS OVERDUE INCLUDING RECITAL, REGISTRATION, DANCEWEAR, SHOES OR ANY OTHER ORDERED SUPPLIES OR APPLIED COSTS. RECITAL PRODUCUTION FEES ARE DUE BY OCT 31ST AND WILL BE AUTO CHARGED BY DEC 18TH IF NOT RECIEVED BY OTHER PAYMENT FORM FOR ALL STUDENTS CURRENTLY ENROLLED. ALL STUDENTS MUST PAY THIS FEE WHETHER THEY PARTICIPATE OR NOT. NO REFUNDS.
